Terms & Conditions

The GoHealthy360 Rewards Program (“Program”) is owned and operated by Liquid Soap Products, LTD. (“Sponsor”) or its third-party providers. The Program is provided subject to compliance with the terms and conditions of the GoHealthy360 Rewards Terms and Conditions (“Terms and Conditions”).

Please read the Terms and Conditions carefully before accessing or using the Program. By accessing or using the Program, you agree to be bound by the Terms and Conditions set forth below, as if you had actually signed them. If you do not agree to the terms and conditions set forth below, you may not use the Program.

You acknowledge and agree that the Terms and Conditions constitute a legally binding contract between you and Sponsor. Sponsor may modify the Terms and Conditions as described below. You agree to review the Terms and Conditions often to familiarize yourself with such modifications. The most current version of the Terms and Conditions is available at gohealthy360.com and will supersede all previous versions. By completing the registration process, you become an authorized user of the Program (a “Member”).

A. Membership

The Program is open only to legal residents of the forty-eight (48) continental states of the United States, eighteen (18) years of age or older. Void where prohibited by law. Membership in the Program, any GoHealthy360 Rewards Points (“Rewards Points”) earned, or any other related membership benefits are not transferable and may not be combined or merged among other Member accounts, their estates, successors or assigns.

B. Program Term

The Program may be terminated by Sponsor at any time for any reason, even though termination may affect a Member’s ability to accrue or redeem Rewards Points. In such event, Sponsor shall post notice of Program termination on the Program website (“Website”), or through other means as determined by Sponsor in Sponsor’s sole discretion, 90 days prior to termination of the Program.

C. Enrollment

Program enrollment is at gohealthy360.com. Member must have a valid, unique email address to create a GoHealthy360 Rewards account (“Account”). Rewards Points will be tracked in Member’s Account by Sponsor. Multiple persons are not permitted to share the same e-mail address or Account. Any attempt to obtain more than the stated number of Accounts by using multiple/different e-mail addresses, identities, registrations and logins, or any other methods may result in disqualification from the Program. In the event of a dispute as to any Account, the individual listed as the primary account contact for the GoHealthy360 account will be deemed to be the Member.

D. Collecting Rewards Points

• Member can receive Rewards Points through the sponsor or third-party providers. Sponsor reserves the right to change the products and services eligible for Rewards Points, and the number of Rewards Points available for qualifying purchases, at any time during the Program.
• Rewards Points are not transferable, and may not be bartered, sold, or redeemed for cash. Member may not combine Rewards Points obtained by others for deposit into a single Member account. Any attempt to combine or transfer Rewards Points may result in disqualification from the Program and forfeiture of all Rewards Points.
• Member is responsible for ensuring the accuracy of Member’s Account.
• Sponsor reserves the right to change, add, or remove the methods by which Members can collect Rewards Points.

E. Expiration of Rewards Points

• A Member will be removed from the Program and forfeit all unused points upon expiration of their membership.
• Any unused Rewards Points shall be forfeited without compensation upon Member’s removal from the Program by Sponsor for any other reason, or upon Sponsor’s termination of the Program.

F. Redeeming Rewards Points

• If Member has achieved sufficient Rewards Points, Member may redeem Rewards Points for merchandise or take advantage of other offers (“Rewards”) as outlined on the Website. Sponsor reserves the right to modify the list of merchandise and other items available for redemption, as well as their corresponding Rewards Points values, at any time for any reason.
• A valid U.S. shipping address (P.O. Boxes not permitted) and telephone number is required to redeem Rewards Points. Allow approximately 1 to 3 weeks for delivery following redemption.
• All valid redemptions for Rewards are final. Rewards Points shall immediately be deducted from Member’s account upon the valid redemption of a reward through the Website.
• Rewards Points have no cash value and may only be used to redeem Rewards offered in the Program.

G. General Terms and Conditions

• Sponsor is not responsible for incorrect or inaccurate transcription of information, technical failures, or any error or malfunction related to the Program. To the fullest extent permitted by law, Sponsor disclaims liability for any direct, indirect, incidental, special or consequential damages arising out of the Program.
• There are links on the Site that allow Member to visit sites of third parties. These sites are not controlled by Sponsor, and Sponsor makes no representations concerning the information, products, or services offered on such sites.
• Any and all disputes shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate Missouri State Court located in Jackson County, Missouri. These Terms and Conditions are governed by the laws of the State of Missouri.
• By participating, Member agrees to release and hold harmless Sponsor and its affiliates from any losses, damages, claims, or causes of action arising out of participation in the Program.
• Members are responsible for the payment of any and all taxes which may result from Rewards received as part of the Program.
• The Site’s forms are protected by Google reCAPTCHA. Use of reCAPTCHA is subject to Google’s Privacy Policy and Terms of Service, as described in Sponsor’s Privacy Policy.
• The terms of Sponsor’s Privacy Policy are incorporated herein. Liquid Soap Products will not distribute or sell any personal information provided by Member and holds all information in the strictest confidence.
• Sponsor reserves the right to remove any Member who engages in fraudulent activity or uses the Program inconsistently with these Terms and Conditions or any applicable laws.
• The Program is provided to individuals only. Corporations, associations, or other groups may not participate.
• If any provision of these Terms and Conditions is found to be invalid or unenforceable, that provision shall be severed and the remainder will remain in full force and effect.
